PERSONAL ACCIDENT POLICY – AIG
COVERAGES (Per Student)
- Accident Medical Expenses: $3,000.00 (Accident in school $1500.00).
- Accident Deductible: $40.00
- Ambulance limit: $500.00
DEFINITION OF ACCIDENT:
Accident is understood to be the event due to unforeseen, fortuitous and occasional causes of an external force that acting suddenly on the person of the insured, regardless of his or her will, produces clinically proven bodily injuries, which cause death, total permanent disability or partial, or temporary disability.
ACCIDENT NOTIFICATION:
Give notice of the accident to the General Secretary of the SEK International School – Ecuador, within a period not exceeding eight days after the accident, otherwise it will not be covered. Estimated processing time from the presentation of the documents is 20 days.
CLAIM PROCEDURE:
a) MEDICAL EXPENSES PER ACCIDENT:
In case of an accident, the insured student may be treated at any clinic or hospital of his/her choice and once the outpatient or inpatient, clinical or surgical treatment has been completed, the student or his/her representative must submit the following documents to the General Secretary's Office in order to process the reimbursement of the expenses incurred:
Medical Expense Claim Form for processing under reimbursement:
- Personal Accident claim form duly filled out.
- Form number 008 or emergency form (Mandatory to bring it).
- Original or electronic invoices valid for tax purposes (not receipts).
- Original medical prescriptions.
- Medical examination orders.
- Original invoices corresponding to the exams.
- Detail values of invoices.
- Legal Representative's bank account information (Name of the Bank/ Account number/ Account type) and copy of ID.
1. Outpatient treatment:
- Original invoices for medical fees.
- Original prescriptions.
- Original invoices from pharmacies (print out -electronic invoice).
- Medical examination orders.
- Original invoices corresponding to exams.
2. Hospital Treatments:
- Original bill from the clinic or hospital.
- Copy of the Medical Record.
- Invoice breakdown.
- RX plates and report.
b) IN CASE OF STUDENT DEATH:
- Original birth certificate or copy of identity card.
- Original death certificate.
- Autopsy protocol.
- Body removal act.
- Any other documents that the company will need.
A copy of the identity card of the legal representative, to whom the refund cheque will be issued.
